The prompt
“If you got to invent a whole new world from scratch, what's the first thing you'd put in it?”
What to listen for
If they start with something fun and whimsical (candy rivers, floating islands), that's just healthy imagination at play, enjoy it. If they start with rules or people (everyone is kind, no one is left out, everyone has a home), that's a hint about what they wish was more true in the real world.
If their world includes you or the family prominently, that's a sign of how central home feels to their sense of security even in fantasy. If their world is entirely about escape, no rules, no grown-ups, that's also normal and can simply mean they're craving more freedom or control somewhere in daily life.
If they say "I don't know," ask what would NOT be allowed in their world, sometimes the "nots" come easier than the "haves."
What this tells you
The very first thing they add to an imaginary world usually points at what they feel is missing or want more of in the real one.
